These fields are automatically populated with the details of both the internally assigned person responsible for the assessment and the external medical examiner/evaluator/assessor that will be performing the work. Details of the individuals are stored in their profiles.

The data stored in these fields are used to auto-populate Part 4 of the OCF-25 and OCF-26 forms.
