Adding Registration Questions
From SmartWiki
Next you will add the registration page and questions.
1. Click the Edit button.
The Form Questions panel is displayed.
2. Click the New Page tab.
The New page is displayed.
3. Set the Page Setting as shown below:
4. Click the Save button.
The page is added to the form.
You will now add some questions to the form.
5. Add the following four questions:
The question list should look as follows:
